General Clinic Questions

What services do you offer?
At Haven Health, we focus on compassionate, evidence-informed evaluations for Utah’s medical cannabis program. We offer:

  • Initial evaluations for new patients

  • Renewal visits for existing cardholders

  • Premium visits where we assist with the entire state application process during your appointment

All visits are with a board-certified physician who’s been helping Utah patients navigate medical cannabis since 2020.

Where are you located, and how do I make an appointment?
We’re based in St. George, Utah. Appointments can be booked online —it only takes a minute. You’ll receive a confirmation and any forms you need to complete beforehand.

Do you offer telehealth visits?
Yes, telehealth is available for renewal visits. New patients or those transferring care will need to be seen in person per Utah law. If you’re not sure what you qualify for, we’ll guide you through it.

Payments & Insurance

Do you accept insurance?
No. We are a cash-based clinic and do not bill insurance at this time. Payment is due at the time of service. This allows us to spend more time with you and offer a more personalized, direct-care experience.

What forms of payment do you accept?
We accept all major credit and debit cards, as well as Apple Pay. We do not accept cash or checks at this time.

Can I use my HSA or FSA card?
Many patients are able to use HSA/FSA cards for medical cannabis evaluations, but we recommend checking with your benefits provider to be sure. We’re happy to provide a receipt if you need one for documentation.

Do you offer refunds?
We do not offer refunds once a visit has occurred. If you need to cancel or reschedule, we ask for at least 24 hours’ notice to avoid a late cancellation or no-show fee.

Medical Cannabis Program

Who qualifies for a medical cannabis card in Utah?
Utah’s program allows for medical cannabis use for a range of qualifying conditions—including chronic pain, PTSD, cancer, and others. You can find the full list here: [link to Utah DOH site]. If you're not sure whether you qualify, feel free to book a visit or reach out with questions.

How do I apply for a medical cannabis card?
You’ll need to create a Utah ID account and complete the state’s online application. We’ll then certify you in the system after your visit. If you'd rather not deal with the paperwork, book a Premium Visit and we’ll take care of it for you during your appointment.

Can I transfer my care to you if I’ve seen another provider?
Yes. If your card was issued by a different Qualified Medical Provider (QMP), we can take over your care. However, Utah law requires that we see you in person for your first visit with us, regardless of whether you're getting a new card or renewing an existing one.

Do I need to be seen in person?
If you are a returning Haven Health patient, you may be eligible for a telehealth renewal.
If you are new to Haven Health—even if you’re renewing a card issued by another provider—you’ll need to be seen in person first.

Visit Types

What is a Premium Visit?
A Premium Visit is ideal if you want extra time and help navigating the application process. During this visit, we’ll walk you through every step, complete your state application together, and ensure everything is submitted before you leave. It’s perfect for first-time patients or anyone who just wants a little extra support.

What’s the difference between an initial consult and a renewal?
Initial consults are for first-time applicants or those whose cards have expired. Renewals are shorter visits for patients with an active card.
Important: If this is your first visit with Haven Health, you’ll need to be seen in person—even if your card is still active or you’ve had a renewal before with another provider.

How often do I need to renew my card?
In Utah, medical cannabis cards are typically valid for up to a year. We’ll let you know when you’re due and make the renewal process easy when the time comes.

Policies

What is your cancellation/no-show policy?
We ask for at least 24 hours’ notice if you need to cancel or reschedule. No-shows or late cancellations may be charged a $50 fee. This helps us keep our schedule available for other patients who need care.

What if I’m late to my appointment?
If you’re running late, we’ll do our best to accommodate you, but your appointment may need to be rescheduled if there’s not enough time to complete the visit.

Do you provide medical records or documentation upon request?
Yes. If you need a copy of your visit notes or a summary for your records, just let us know. We’ll get it to you promptly—no extra charge.
