Do You Qualify?

For some conditions, medical cannabis may be a reasonable treatment option. Utah law allows medical cannabis for many common conditions. You may qualify if you are 21 years old and have:

  • acute pain for a condition such as surgery

  • chronic or persistent pain that lasts longer than 2 weeks.

  • post-traumatic stress disorder (PTSD) that you’re also getting treatment for from a mental health provider. 

  • Crohn’s disease or ulcerative colitis

  • Alzheimer's disease

  • autism

  • cancer

  • Weight loss due to serious illness (cachexia)

  • nausea that’s not caused by pregnancy or cannabis-related conditions

  • epilepsy or seizures

  • multiple sclerosis (MS) or muscle spasms

  • HIV or AIDS

  • amyotrophic lateral sclerosis

  • a rare condition or disease defined by the National Institutes of Health

  • a terminal illness or hospice care with a life expectancy of less than 6 months.

Create an account on the Utah Department of Health’s Electronic Verification System (EVS).

From there, you can submit a new application or renew your medical cannabis card. The process is secure and typically takes just a few minutes.

Once your application is started, schedule a visit with Dr. Haberle, a board-certified internal medicine physician with experience guiding Utah patients through medical cannabis certification since 2020.

Visits are available in person or via telehealth, depending on your needs. You’ll have the chance to discuss your medical history, explore whether cannabis may help, and get straightforward answers to your questions — without judgment or pressure.
